Job Description
- Reviews referrals and assesses participant’s eligibility for admission in program.
- Completes full nursing assessment and develops medical plan for each participant.
- Assists treatment team in development and implementation of overall treatment plan.
- Actively participates as a treatment team member and performs related tasks.
- Provides medical case management including coordination of services with medical director, medical providers and other related service providers in the program and community.
- Acts as liaison with physicians, pharmacies and families regarding medical issues.
- Performs weekly rounds at program and more frequently as needed.
- Monitors medication administration and trains staff on medication handling, distribution and related policies and procedures.
- Trains and oversees medication coordinators.
- Assists with ensuring compliance with contractual, legal, and regulatory requirements.
- Performs all duties inherent in a supervisory role.
- Performs other duties, as assigned.
Qualifications:
- Completed Diploma, Associate, or Baccalaureate Degree in Nursing.
- Maintains unrestricted license to practice as a Registered Nurse in the state(s) to which assigned.
- Maintains current BLS (Basic Life Support) or CPR certification, in accordance state regulations.
- A minimum of two-year’s experience as licensed nurse.
- Case management experience and experience in residential environment preferred.
- Communicates effectively and professionally in verbal and written interactions.
- Demonstrates strong organization skills and attention to detail.
- Has reliable transportation and is able to travel to/from local program and community locations throughout work-day and week.
