Registered Nurses (RNs) to complete
Start of Care (SOC) admission visits throughout
North Delray through Vero Beach .

Job Responsibilities
- Complete Start of Care (SOC) admission visits
- Provide prompt and accurate communication regarding client medical status to the Nurse Supervisor to ensure effective case management
- Travel throughout the Treasure Coast service area as needed
- Demonstrate empathy, professionalism, and confidence when interacting with clients and families
- Maintain thorough, accurate, and timely documentation
- Anticipate potential issues and appropriately resolve or escalate concerns
- Prioritize tasks and demonstrate a sense of urgency
Required Experience & Education - Active Registered Nurse (RN) license (Florida)
- Current Level 2 clearance in the AHCA Clearinghouse
- TB test within the last 12 months or Chest X-ray within the last 5 years
- Prior home care experience required
- Excellent communication, collaboration, and customer service skills
- Ability to use an electronic health record (EHR) for documentation and care coordination
