Westcott Community Center is seeking a compassionate, motivated, and community-minded Nutrition Counselor to help empower our neighbors through improved nutrition, chronic disease support, and access to healthy meals.
This is a 20-hour/week, part-time, remote (with occasional on-site needs) position with flexible scheduling and some evening/weekend availability for outreach and community events.

Location: Remote + local travel to center/events/meetings
Hours: 20 hours/week, flexible
Pay: $40/hour

Reports To: Executive Director & Director of Operations
About the Role:
The Nutrition Counselor plays a key role in our mission to strengthen community health. As a Certified Dietitian/Nutritionist (CDN) or Registered Dietitian Nutritionist (RDN) , you’ll provide individualized nutrition support, deliver education, and connect neighbors with essential health resources—from healthy food access to chronic condition management.

Key Responsibilities:
Case Management & Nutrition Support:
- Accept and manage referrals promptly
- Complete intake assessments and biannual Life Needs Assessments
- Develop individualized nutrition plans and monitor progress
- Conduct HRSN (Health Related Social Needs) screenings for Medicaid members as needed
- Maintain frequent contact with participants and assist with referrals
- Collaborate closely with our Community Health Advocate
- Maintain accurate, confidential client records (HIPAA & Healthy Alliance compliant)
Healthy Food Access & Chronic Disease Guidance:
- Connect eligible participants to our Basic Needs Pantry, Medically Tailored Meals, and Produce Prescriptions
- Provide nutrition support for chronic conditions (diabetes, hypertension, obesity, heart disease, food allergies)
Community Engagement & Education:
- Support community health events and initiatives
- Develop educational materials and occasionally present workshops
- Attend community meetings and represent the center professionally
- Engage in ongoing professional development
Data & Reporting:
- Track services, outcomes, and program data using Unite Us and Salesforce
- Complete documentation in a timely, organized manner
Other:
- Maintain reliable transportation for meetings and on-site needs
- Additional duties as assigned

- CDN or RDN credential (required)
